Please list any medical conditions you may have that may include pre-existing weaknesses, trauma or injury, post-operative recovery from surgery, or pregnancy. Please also advise whether the condition is spinal, neural or muscular. Knowledge of this detail assists the instructors of a ‘Dancescene’ Adult Ballet class in safely accommodating your participation in a class. Knowledge of any pre-existing medical conditions assists the instructors of a Dancescene class to safely accommodate your participation. It is advised that if in any doubt about the suitability of a class for your particular physical or medical condition, that you consult your Doctor first, before commencing classes.

By submitting this registration form you also accept that participation is conditional upon you exercising at a pace that is entirely comfortable for you. Stretching is an individual effort and requires that you not only participate at your own comfort level but that you also communicate with the instructor regarding any possible personal discomfort. If in doubt, it is essential that you cease exercise in the class immediately and consult your medical professional.
